NHTD WAIVER PROGRAM IN Brooklyn

We provide services under Nursing Home Transition and Diversion (NHTD) waiver. Our NHTD program helps clients take advantage of all the benefits it offers while they are living in their own homes, amidst their personal treasures and familiar surroundings.

The NHTD waiver program is a home and community-based program that helps New York´s Medicaid-eligible seniors and people with physical disabilities receive comprehensive services they need while they live in a community-based setting, rather than in a nursing home or other institution.
