- Plan of Care: All services are delivered based on a customized Plan of Care developed in consultation with the client, their family, and medical professionals.
- Service Area: Services are only available within our licensed service areas in New York State.

3. Eligibility and Enrollment
To be eligible for certain services, clients may need to meet specific criteria set by New York State Medicaid, Managed Long Term Care (MLTC) plans, or private insurance providers.
- We reserve the right to refuse service if the home environment is deemed unsafe for our staff.
- Enrollment is finalized only after a formal Service Agreement is signed.
4. Billing and Payment
- Payment Methods: We accept Medicaid, Private Pay, and various Long-Term Care insurance plans.
- Rates: For private pay clients, rates are disclosed prior to the start of service.
- Late Payments: Failure to pay for services rendered may result in the suspension or termination of care, subject to state regulations regarding patient discharge.
5. Cancellations and Termination
- Client Rights: Clients may terminate services at any time. We request 24-hour notice for schedule changes or cancellations to avoid administrative fees.
- Agency Termination: Galaxy Home Care reserves the right to terminate services due to non-payment, safety concerns, or if the level of care required exceeds what the agency can safely provide.
6. Privacy and HIPAA Compliance
Your privacy is paramount. We handle all Personal Health Information (PHI) in strict accordance with the Health Insurance Portability and Accountability Act (HIPAA). Please refer to our Privacy Policy for detailed information on how we protect your data.
7. Limitation of Liability
To the fullest extent permitted by law, Galaxy Home Care shall not be liable for any indirect, incidental, or consequential damage resulting from the use of our website or the provision of services, except in cases of gross negligence or willful misconduct by our staff.
